Emergency Procedures:

A 24/7 emergency telephone assistance service is available for your benefit so that, in the event of an emergency while traveling, English speaking help and advice may be furnished to you. In case of an emergency please contact On Call International as follows:

Claims are handled by Co-ordinated Benefit Plans LLC.

To facilitate prompt claims settlement:
Trip cancellation claims:
Contact Co-ordinated Benefit Plans LLC to obtain the appropriate claim form. Please be sure to include
detailed information supporting the reason for the trip cancellation, and proof of paid and non-refundable
funds that you wish to claim back.
Trip interruption claims:
Contact Co-ordinated Benefit Plans LLC to obtain the appropriate claim form. Please be sure to include
detailed information supporting the reason for the trip interruption, and proof of paid and non-refundable
funds that you wish to claim back. Provide all unused transportation tickets, official receipts, etc.
Claims for medical expenses reimbursement:
Contact Co-ordinated Benefit Plans LLC to obtain the appropriate claim form .Obtain receipts from the
providers of service, etc., stating the amount paid and listing the diagnosis and treatment.
Claims for loss of diving gear/Baggage:
Contact Co-ordinated Benefit Plans LLC to obtain the appropriate claim form. In case of loss, theft, or
damage to diving gear/Baggage, immediately contact the hotel manager, tour guide or representative,
transportation official, or local police; report occurrence and obtain a written statement. Submit claim
first to party responsible; i.e., airline, hotel, etc.
